Introduction

Localized fat does not always respond completely to diet, exercise, or weight loss. Even in people who maintain a healthy weight and active lifestyle, small resistant fat deposits may persist in specific areas and affect body or facial contour.

In some patients, fibrosis and changes in the surrounding connective tissue may also contribute to firmness, irregularity, or an unwanted contour. The concern, therefore, is not always simply excess fat — the characteristics of the tissue itself can also be important.

pbserum Lipase & Recombinant Enzyme Treatment offers a minimally invasive injectable approach for selected patients with localized fatty and fibrotic tissue. For localized fatty fibrosis, pbserum HA Medium combines Lipase PB500, Collagenases G/H PB220, and Lyase PB72K in a formulation emphasizing lipase activity.

At Cabo Plastic Surgery, treatment is individualized according to the anatomy, tissue characteristics, skin quality, and the patient's aesthetic goals. The objective is not weight loss or large-volume fat reduction, but targeted improvement of selected areas where localized fat, fibrosis, or both contribute to an unwanted contour. Patients requiring more substantial fat reduction may be better candidates for liposuction or another body-contouring procedure.

Successful body contouring is not about treating every area — it is about selecting the right treatment for the right tissue.

What Is pbserum Lipase & Recombinant Enzyme Treatment?

pbserum HA is an injectable medical-device system that uses recombinant enzymes in different proportions according to the characteristics of the tissue being treated. For localized fatty fibrosis, pbserum HA Medium combines three recombinant enzymes and contains a higher proportion of Lipase PB500.

Lipase PB500

The principal enzyme within HA Medium for addressing the adipose component of the treatment area. According to pbserum, Lipase PB500 acts on triglycerides within adipose tissue, breaking them down into smaller components that can subsequently be processed by the body. The objective is a localized reduction in adipose-tissue volume, not generalized weight loss.

Collagenases G/H PB220

Fibrosis involves changes in the organization and accumulation of collagen within the tissue and can contribute to firmness, tethering, irregularity, and altered contour. Collagenases G/H PB220 are designed to act on altered or non-functional collagen within fibrotic tissue, supporting remodeling of the extracellular matrix.

Lyase PB72K

Complements the actions of lipase and collagenase by acting on components of the extracellular matrix. According to pbserum, this enzymatic activity can reduce extracellular-matrix viscosity and support tissue permeability and fluid drainage.

Why combine different enzymes?

Localized contour concerns may involve more than excess fat. Fibrosis and changes within the surrounding extracellular matrix can also influence tissue firmness, texture, and contour. By combining enzymes with different functions, pbserum HA Medium is designed to address these tissue components as part of a targeted treatment approach.

Benefits of pbserum Recombinant Enzyme Treatment

When appropriately selected and performed, treatment may provide several benefits:

  • Targeted treatment of selected localized fat deposits.
  • Improvement in areas where fibrosis contributes to contour irregularity.
  • Progressive improvement in tissue contour and definition.
  • Treatment tailored to the characteristics of the tissue.
  • Minimally invasive treatment without surgical incisions.
  • Limited downtime compared with surgical body contouring.

The treatment is intended for relatively modest, localized correction rather than major body reshaping. Patients with larger fat deposits, significant skin excess, or more substantial contour concerns may be better candidates for another treatment or surgical procedure.

Treatment Areas

Treatment may be considered for selected facial and body areas where localized fatty or fibrotic tissue contributes to an unwanted contour. Potential treatment areas may include:

  • Submental area — selected fullness beneath the chin.
  • Lower face — carefully selected localized fatty tissue affecting facial contour.
  • Upper arms.
  • Abdomen and flanks.
  • Back and bra-line area.
  • Hips and thighs.
  • Area around the knees.

Treatment suitability depends on the anatomy and characteristics of each area. Not every area of fullness is caused primarily by localized fat; loose skin, muscle anatomy, skeletal structure, generalized fat accumulation, or another underlying condition may be responsible. A physical examination is therefore important before determining whether injectable enzyme treatment is appropriate.

Facial vs. body treatment

Facial treatment requires particular caution because even small changes in volume can significantly affect facial proportions. Reducing volume from an inappropriate area may contribute to an aged or unbalanced appearance.

Treatment of the face should therefore be based on careful evaluation of facial anatomy, existing volume, skin quality, and the anticipated effect of reducing a particular area of fullness. The objective is not simply to make the face thinner, but to determine whether reducing a specific area of localized fullness will improve facial balance.

Localized Fat, Fibrosis & Tissue Quality

Successful contouring begins with identifying what is actually contributing to the area of concern. Localized fat, fibrosis, and skin laxity are different problems and may require different treatment approaches.

Localized fatRelatively limited adipose deposits that persist in specific areas despite reasonable weight control. Genetics, age, hormonal influences, and individual patterns of fat distribution all affect where the body preferentially stores fat. Injectable enzyme treatment is most appropriate when the amount of unwanted fat is relatively limited and the desired correction is correspondingly modest. It is not a treatment for obesity or a substitute for weight loss.
FibrosisChanges in the organization of collagen and connective tissue. Affected tissue may feel firmer or less mobile and can contribute to visible irregularities in contour or texture. When fibrosis is part of the problem, reducing fat alone may not fully address the tissue characteristics responsible for the contour concern.
Skin quality & laxityReducing localized fat does not automatically tighten loose skin. In patients with limited skin elasticity, decreasing underlying volume may make existing laxity more apparent. When skin laxity or another anatomical factor is the primary concern, a different treatment may provide a better result.

At Cabo Plastic Surgery, the goal is not simply to reduce fat, but to improve contour while respecting the tissues and anatomy that create it.

Ideal Candidates

You may be a good candidate if you

  • Have a relatively small, localized area of resistant fat or fatty fibrosis.
  • Are at or reasonably close to a stable, healthy weight.
  • Have adequate skin quality and elasticity in the treatment area.
  • Prefer a minimally invasive treatment with limited downtime.
  • Understand that treatment is intended for contour improvement, not weight loss.
  • Have realistic expectations about the degree of improvement possible.

Another approach may be better when

  • Fat accumulation is larger or more diffuse.
  • Significant loose or excess skin is present.
  • The contour concern is primarily related to muscle or skeletal anatomy.
  • A more substantial or predictable contour change is desired.
  • A medical condition or active problem makes treatment inappropriate.
  • You are pregnant or breastfeeding, or have active infection, inflammation, or compromised skin in the proposed treatment area.

Ultimately, candidacy depends not simply on whether an area can be treated, but whether injectable enzyme treatment is likely to provide a meaningful improvement for that particular patient.

Your Consultation & Personalized Treatment Plan

Successful treatment begins with determining what is actually creating the contour concern. During your consultation, Dr. Roberto López Konschot will evaluate:

  • The location and amount of localized fat.
  • Presence and degree of fibrosis.
  • Skin quality, elasticity, and existing laxity.
  • Facial or body anatomy and proportions.
  • Previous surgery or aesthetic treatments.
  • Medical history, medications, and weight stability.
  • Your desired degree of correction.

Standardized photographs may be taken to document the treatment area and evaluate changes over time.

Choosing the appropriate treatment

The presence of localized fat does not automatically mean that pbserum HA Medium — or any injectable treatment — is the best choice. Depending on your anatomy and goals, the most appropriate approach may involve injectable recombinant enzyme treatment, another non-surgical treatment, liposuction, skin-tightening or body-contouring surgery, or a combination of treatments.

Setting realistic expectations

Injectable recombinant enzyme treatment is best suited to relatively limited, targeted contour correction. It should not be expected to reproduce the degree of fat removal achievable with liposuction. For the appropriately selected patient, however, a more limited correction may be exactly what is needed.

What to Expect During Treatment

pbserum HA Medium is administered by injection into the selected treatment area according to an individualized medical protocol.

Before treatment

  • The treatment area is examined and the planned treatment zones are identified.
  • The skin is cleansed and prepared, and photographs may be taken for comparison.
  • Appropriate measures may be used to make the procedure more comfortable.

During treatment

  • The recombinant enzyme preparation is administered through multiple carefully planned injections.
  • The amount, distribution, depth, and injection pattern are determined according to your anatomy, tissue characteristics, degree of fibrosis, and treatment objectives.
  • You may feel brief pinching, pressure, stinging, or tenderness.

Immediately after

  • Temporary swelling, redness, tenderness, bruising, or local discomfort may occur.
  • The treated area may initially appear more swollen rather than smaller, so the immediate appearance should not be interpreted as the final result.

Recovery & Aftercare

Recovery is generally shorter than after surgical body contouring. The degree and duration of post-treatment effects vary according to the area treated, amount of product used, tissue characteristics, and individual response.

Common temporary effects

  • Swelling.
  • Redness.
  • Tenderness or sensitivity.
  • Bruising.
  • Mild burning or stinging.
  • Temporary firmness or tightness.
  • Localized discomfort at the injection sites.

Aftercare

  • Keep the treated area clean.
  • Avoid unnecessary pressure or manipulation unless specifically instructed.
  • Avoid strenuous exercise until significant swelling or discomfort has improved.
  • Avoid excessive heat, saunas, steam rooms, and hot tubs during the initial recovery period.
  • Avoid other aesthetic treatments in the same area until cleared by the clinic.
  • Follow any individualized instructions regarding compression, massage, skincare, or activity.

Contact Cabo Plastic Surgery if you develop increasing or severe pain, marked or rapidly increasing swelling, significant skin discoloration, blistering or skin breakdown, spreading or worsening redness, drainage or signs of infection, fever or systemic symptoms, or any unexpected or concerning reaction. Any symptom that is severe, progressive, or unusual should be evaluated promptly.

Results

Results develop progressively. The objective is not immediate or large-volume fat reduction, but improvement in the contour of a selected area by addressing the localized fatty and fibrotic tissue contributing to its appearance. Depending on the treatment area and individual response, patients may notice:

  • Reduction in localized fullness.
  • Improved contour and definition.
  • Improvement in tissue firmness.
  • Smoother appearance of selected irregularities.
  • Improvement in areas where fibrosis contributes to the contour concern.

When will I see results?

Some patients may begin to notice changes after an initial treatment, but results should be evaluated progressively rather than immediately. Early swelling can temporarily obscure improvement. As the initial treatment response settles and tissue remodeling progresses, changes in contour become easier to evaluate.

How much improvement can I expect?

The degree of improvement depends on the amount and location of localized fat, degree of fibrosis, skin quality and elasticity, size of the treatment area, number of treatments performed, individual biological response, and your starting anatomy and treatment goals.

Patients with relatively small, well-defined areas of localized fatty or fibrotic tissue are generally better suited to this treatment than patients seeking substantial fat reduction.

Treatment Protocol & Maintenance

There is no single treatment schedule appropriate for every patient. The number and timing of sessions are individualized according to the treatment area, amount of localized fatty tissue, degree of fibrosis, skin quality, tissue response, and desired degree of correction.

Why might more than one treatment be recommended?

Injectable recombinant enzyme treatment is intended to create progressive change rather than aggressive correction in a single session. After each treatment, the area is allowed to respond before determining whether another session is appropriate. Treatment should stop when the desired improvement has been achieved, when further treatment is unlikely to provide meaningful benefit, or when another approach would be more appropriate.

Maintaining your results

Long-term contour is influenced by weight changes, aging, lifestyle, and individual anatomy. Maintaining a relatively stable weight, regular physical activity, and healthy lifestyle can help preserve the overall result.

Future treatment is not automatically necessary simply because time has passed. If a new or recurrent contour concern develops, the area should be reassessed before deciding whether additional treatment — or a different approach — is appropriate.

Complementary Procedures

This treatment addresses relatively localized contour concerns. When other anatomical factors contribute to the problem, another treatment may be recommended instead of — or in combination with — injectable enzyme treatment.

LiposuctionWhen greater fat reduction or a more substantial and predictable contour change is desired, liposuction may be the more appropriate option.
Skin-tightening treatmentsWhen skin laxity contributes significantly to the contour concern, a treatment directed specifically at skin quality or tightening may be considered.
Abdominoplasty & body-contouring surgeryWhen excess skin, muscle separation, and more substantial fat deposits are present, particularly in the abdomen, abdominoplasty or another body-contouring procedure may provide a more complete correction.
Facial rejuvenation treatmentsIn selected facial areas, treatment planning may also include procedures that address skin quality, volume distribution, or facial aging. Because reducing facial fat can alter facial proportions, treatment should always be planned in the context of the face as a whole.

Not every treatment should be performed at the same time. When multiple procedures are appropriate, their sequence and timing are individualized. Combination treatment is recommended only when each procedure has a clear purpose.

Risks & Safety

pbserum HA Medium is an injectable medical device and should be administered by an appropriately qualified medical professional familiar with the product, injection technique, and anatomy of the treatment area. As with any injectable treatment, temporary local reactions can occur.

Expected or common temporary reactions

  • Pain or tenderness at the injection sites.
  • Redness.
  • Swelling or localized edema.
  • Bruising.
  • Itching or irritation.
  • Temporary firmness or sensitivity.

Less common or potential complications

  • More persistent inflammation.
  • Allergic or hypersensitivity reactions.
  • Rash or urticaria.
  • Infection.
  • Prolonged swelling or discomfort.
  • Changes in skin appearance or contour.
  • An unexpected or excessive local tissue response.
  • Vasovagal reactions associated with injections, discomfort, or anxiety.

When treatment should be avoided or postponed

  • Pregnancy or breastfeeding.
  • Known allergy or hypersensitivity to components of the formulation.
  • Active infection in or near the treatment area.
  • Significant active inflammation or compromised skin.
  • Certain active autoimmune conditions.
  • Active oncologic disease.
  • Other medical conditions that may interfere with safe treatment or healing.

The safety of injectable contouring depends not only on the product, but also on appropriate patient and product selection, accurate diagnosis, knowledge of anatomy, correct injection technique and dosing, sterile technique, and appropriate follow-up. More product or more aggressive treatment does not automatically produce a better result.

Why Choose Cabo Plastic Surgery?

Effective non-surgical contouring requires more than selecting an injectable product. It requires understanding the anatomy, the characteristics of the tissue being treated, and what a minimally invasive treatment can realistically accomplish.

Treatment planning is guided by Dr. Roberto López Konschot, a board-certified plastic surgeon with more than 20 years of surgical experience and advanced training in facial aesthetics and craniofacial surgery. Our approach includes:

  • Individualized evaluation of localized fat, fibrosis, skin quality, and laxity.
  • Careful consideration of facial or body anatomy and proportions.
  • Conservative patient and treatment selection.
  • Realistic discussion of expected improvement and limitations.
  • Consideration of surgical alternatives when more appropriate.
  • Individualized treatment planning, follow-up, and aftercare.
  • Honest recommendations when treatment is unlikely to provide meaningful benefit.

Because both non-surgical and surgical options can be considered, the objective is not simply to offer the least invasive treatment. It is to recommend the treatment most appropriate for your anatomy and goals.

Frequently Asked Questions

What is pbserum Lipase & Recombinant Enzyme Treatment?

A minimally invasive injectable treatment using recombinant enzymes to address selected localized fatty and fibrotic tissue. For localized fatty fibrosis, pbserum HA Medium combines Lipase PB500, Collagenases G/H PB220, and Lyase PB72K.

Is pbserum HA Medium injectable?

Yes. pbserum HA Medium is an injectable medical device. This page refers specifically to the injectable pbserum HA system, not the company's topical cosmetic enzyme products.

What does Lipase PB500 do?

Lipase PB500 addresses the adipose component of the treatment area and is intended to reduce localized adipose-tissue volume. It is used for localized contour improvement, not weight loss.

Why are collagenase and lyase included?

Localized contour concerns may involve fibrosis and changes in the surrounding tissue in addition to fat. Collagenases PB220 address the fibrotic component, while Lyase PB72K acts on components of the extracellular matrix.

What areas can be treated?

Selected facial and body areas may be treated when localized fatty or fibrotic tissue contributes to the contour concern. Suitability depends on the anatomy, tissue characteristics, skin quality, and treatment goals.

Can pbserum be used for a double chin?

In appropriately selected patients, it may be considered for localized fat beneath the chin. Because submental fullness can have several causes, an examination is necessary before recommending treatment.

How many treatments will I need?

Treatment is individualized. Some patients may achieve an appropriate result with a limited treatment course, while others may benefit from additional sessions. The area is reassessed before determining whether further treatment is appropriate.

When will I see results?

Results develop progressively. Early swelling may temporarily obscure improvement, so changes are better evaluated after the initial treatment response has settled.

Is there downtime?

Temporary swelling, tenderness, redness, bruising, firmness, or discomfort may occur. Recovery varies according to the treatment area and individual response.

Is pbserum the same as other fat-dissolving injections?

No. "Fat-dissolving injections" is a broad term that includes different products and formulations. pbserum HA Medium is a specific recombinant enzyme system designed for localized fatty fibrosis.

Can pbserum replace liposuction?

Not when substantial fat removal or a major contour change is required. pbserum is better suited to selected localized concerns where a more limited, non-surgical correction is appropriate.

Are the results permanent?

Results should not be considered simply permanent. Long-term contour can be influenced by weight changes, aging, skin laxity, lifestyle, and individual biology. Maintaining a stable weight can help preserve the result.

Can pbserum be combined with other treatments?

Yes, when appropriate. Other treatments may be recommended to address skin quality, laxity, or another component that injectable enzyme treatment does not correct. The combination and timing are individualized.

How do I know if this treatment is right for me?

An individualized evaluation can determine what is contributing to the area of concern and whether pbserum is likely to provide meaningful improvement. If another non-surgical treatment or surgical procedure is more appropriate, that option may be recommended instead.

Understanding the evidence

Published clinical research supports the potential use of recombinant enzyme therapy for selected aesthetic concerns involving localized adipose tissue and fibrosis. However, the evidence remains developing. Available studies include relatively small patient populations, different enzyme combinations, formulations, anatomical areas, and treatment protocols. Some published studies have also evaluated enzyme combinations that are not identical to the current pbserum HA Medium formulation. These studies should therefore not be interpreted as proof that every patient, treatment area, or treatment protocol will produce the same result.
