What Are Hyaluronic Acid Fillers & How Do They Work?

Hyaluronic acid is a substance naturally present in the body, including the skin and connective tissues. It has the ability to bind water and plays an important role in tissue hydration and volume. Hyaluronic acid dermal fillers are injectable gels specifically formulated for use in different areas and tissue depths of the face.

Not all fillers are the same. Under-eye area

Filler may be considered for carefully selected patients with appropriate under-eye hollowing. This is an anatomically delicate area, and not every patient with dark circles or a tired appearance is a good candidate. Lower-eyelid bags, skin laxity, pigmentation, and fluid retention may require a different approach, such as blepharoplasty.

Non-surgical nose contouring

In selected patients, filler may be used to improve certain nasal contour irregularities or proportions without surgery. Facial Balance, Volume & Proportion

Successful filler treatment is not simply about adding volume to an individual line or feature. The face is a three-dimensional structure in which the cheeks, lips, chin, jawline, temples, nose, and surrounding soft tissues influence one another. A change in one area can affect how other features are perceived. For this reason, treatment planning begins by looking at the face as a whole.

Restoring volume vs. adding volume

With aging, changes in facial fat, bone, skin, and soft-tissue support can alter facial contours and proportions. In selected areas, carefully placed filler can restore some of this lost support or create smoother transitions between facial regions. This is different from simply making the face fuller. The objective is to determine where volume has been lost, where structural support may be beneficial, and where adding filler would not improve the face.

Enhancement in younger patients

Not every filler patient is seeking age-related rejuvenation. A gradual approach

Previous filler

If you have previously received dermal filler, it is important to tell us what product was used, where it was injected, approximately how much was used, and when it was performed, whenever that information is available. What to Expect During Treatment

Hyaluronic acid filler treatment is performed in the office and generally takes approximately 30–60 minutes, depending on the areas being treated and the complexity of the treatment plan.

Before treatment

  • Your treatment plan is reviewed and the areas to be treated are carefully assessed.
  • Clinical photographs may be taken to document your starting point.
  • The skin is cleansed and prepared; topical or local anesthetic may be used.

During treatment

  • Filler is introduced in carefully selected amounts and anatomical locations.
  • You may feel brief pressure, pinching, or tenderness.
  • Your facial proportions are assessed throughout, so additional product is added only when it provides a meaningful benefit.

Immediately after

  • The initial change is usually visible immediately, but this is not the final result.
  • Temporary swelling, redness, tenderness, injection marks, or bruising can alter the appearance during the first several days.
  • Lips in particular may appear fuller immediately after treatment because of swelling.

Recovery & Aftercare

Most patients experience relatively little downtime after hyaluronic acid filler treatment and can return to normal, non-strenuous daily activities shortly afterward. Temporary injection-related effects are common and may include swelling, redness, tenderness, minor bruising, pinpoint injection marks, temporary firmness or small areas of irregularity, and mild asymmetry related to swelling.

The degree of swelling varies according to the treatment area, amount of product used, injection technique, and individual response. The lips and under-eye area may develop more noticeable swelling than some other facial areas.

Aftercare

  • Avoid rubbing, massaging, or applying unnecessary pressure to treated areas unless specifically instructed.
  • Avoid strenuous exercise during the early recovery period.
  • Avoid excessive heat exposure, including saunas and steam rooms, while significant swelling or redness is present.
  • Avoid swimming in pools or the ocean, as well as hot tubs and Jacuzzis, during the early healing period or until injection sites have closed.
  • Avoid unnecessary pressure from facial treatments or massage during the early recovery period.
  • Use appropriate sun protection.
  • Follow any area-specific skincare and aftercare instructions provided to you.

Recovery timeline

Immediately

  • The result is visible, although swelling, redness, tenderness, or injection marks may temporarily affect appearance.

First 24–48 hours

  • Swelling may become more noticeable before beginning to settle. Bruising may appear.

First few days

  • Most early injection-related effects progressively improve.

1–2 weeks

  • Most swelling and bruising have typically improved substantially, allowing a more accurate assessment of the result.

Recovery varies between patients and treatment areas. Some temporary effects may last longer.

When Should I Contact the Clinic?

Most post-treatment effects are mild and temporary. However, certain symptoms after filler injection require immediate medical attention. Contact Cabo Plastic Surgery immediately if you develop:

  • Severe or increasing pain that seems unusual.
  • Skin that becomes unusually pale, white, gray, dusky, mottled, or progressively discolored.

  • Sudden severe headache, weakness, difficulty speaking, facial drooping, confusion, or other neurological symptoms.

Results

One of the advantages of hyaluronic acid filler is that improvement is generally visible immediately. However, the appearance immediately after injection is not the final result. Swelling, tenderness, and minor bruising can temporarily exaggerate volume or create small differences between the two sides of the face. As these effects settle, the result becomes easier to evaluate.

Depending on the treatment plan, results may include restoration of selected facial volume, improved facial proportions, better definition of selected contours, softer transitions between facial regions, subtle enhancement of the lips, chin, cheeks, or jawline, softening of selected folds or depressions, improved symmetry in appropriate cases, and a more rested or balanced appearance.

The goal is balance, not perfection. Natural faces are not perfectly symmetrical, and successful filler treatment does not require eliminating every line, hollow, or difference between the two sides of the face. You should still look like yourself — only more balanced, refreshed, or refined.

How Long Do Hyaluronic Acid Fillers Last?

The visible aesthetic effect of hyaluronic acid fillers changes over time, but there is no single duration that applies to every product, treatment area, or patient. Longevity can be influenced by:

  • The specific filler used.
  • The area treated.
  • The amount of product placed.
  • The depth and location of injection.
  • Facial movement.
  • Individual metabolism and tissue characteristics.
  • Previous filler treatments.

Areas with greater movement may behave differently from areas where filler is used primarily for structural support. Research using imaging has also shown that residual hyaluronic acid filler may sometimes remain detectable in tissues longer than the visible aesthetic correction would suggest. For this reason, previous filler should always be considered before additional product is placed.

Maintenance

Maintenance should be individualized. Some patients prefer periodic treatment to maintain a particular result, while others return only when they notice that enough of the correction has diminished to make additional treatment worthwhile. Before recommending additional product, the face should be reassessed to determine what filler remains, whether facial proportions have changed, and whether more treatment would actually improve the result. In some patients, the appropriate recommendation may be to wait.

Can hyaluronic acid filler be dissolved?

One advantage of many hyaluronic acid fillers is that they can be reduced or dissolved with an enzyme called hyaluronidase when medically appropriate. Hyaluronidase may be used in selected situations involving an unwanted aesthetic result, excessive or misplaced product, certain filler-related complications, or when existing filler interferes with a new treatment plan.

Dissolving filler is itself a medical procedure and should not be considered a casual "undo button." The need for treatment, amount of hyaluronidase, and expected outcome should be evaluated individually.
